Prescription Drugs

Drug Formulary

The drug formulary is a list of prescription drugs approved under your insurance plan. Be sure to share the formulary with your physician. Using drugs on the formulary can help you save money. The formulary is modified each year on January 1.

Texas Groups: For fully insured employer groups issued in Texas, plan members will continue to use the formulary that was in effect on the date the group was effective until the next renewal date versus automatically switching on January 1.

Prior Authorization and Quantity Limits

Some prescription drugs require prior authorization before your insurance plan will honor the claim. Some have quantity limit designations set by the manufacturer or the Food and Drug Administration. These programs are in place to encourage appropriate use of medications that have certain risk factors.

For plan members with chronic diseases or transplant conditions, our network of national and regional suppliers provides special equipment and medications.
